Dr. Chad Burnham is a physical therapist, strength coach, and the founder of ProForm Physical Therapy in Salisbury, Massachusetts, a performance-based physical therapy clinic serving the North Shore including Newburyport, Amesbury, and surrounding communities.
Dr. Chad’s passion for fitness, strength training, and human performance began in the gym at an early age and continued through his college career. He earned his Doctor of Physical Therapy (DPT) degree from the University of Rhode Island in 2009 and has spent his career helping active individuals move better, train harder, and stay pain-free.
As a Certified Strength and Conditioning Specialist (CSCS), Dr. Chad specializes in bridging the gap between physical therapy, strength training, and performance coaching. His clinical expertise includes sports physical therapy, running injury rehabilitation, golf performance training, advanced manual therapy, and 3D Chain Reaction biomechanics.
After years working in traditional insurance-based clinics, Dr. Chad saw firsthand how the healthcare system often limited the quality of care patients received. Appointments were rushed, treatment plans were restricted, and true one-on-one care became increasingly rare.
Believing patients deserved a better experience, he founded ProForm Physical Therapy in 2016, a cash-based physical therapy clinic in Salisbury, MA focused on individualized care and performance-driven rehabilitation. At ProForm, every patient receives one-on-one physical therapy designed to help them recover from injury, improve strength, and return to the activities they love.Â
Today, Dr. Chad works with athletes, runners, golfers, and active adults across the North Shore of Massachusetts, helping them overcome pain, improve movement, and build long-term resilience.
ProForm Physical Therapy is far from a typical clinic and Dr. Chad Burnham is far from a typical physical therapist.
